It contains two critical ingredients: honesty and courage. I wonder if this applies to the Penn State scandal--as well as also to me...? I work hard at always being honest, but likely I don't speak up, challenge, question (i.e. COURAGE) nearly as often, not wanting to rock the boat or ruffle anyone's feathers. If I have been honest, isn't that enough? Or so I can easily reason. That seems to be the failure here, too, don't you think? While the abuse was reported up the chain, an honest step, it was clearly too easy to stop there and not take the second--but essential--courageous step to acting with full integrity. Courage would have been required to make sure that the reported incidences were then dealt with seriously and appropriately.
